Position: Memory Care Director, LPN

Job Description: We are looking for an experienced and compassionate Memory Care Director to join our new community. The successful candidate will be an LPN responsible for overseeing the care of our residents, ensuring that all needs are met and providing support to our team of caregivers. The ideal candidate will have a background in healthcare, dementia care, and a passion for providing excellent customer service. The Memory Care Director will be responsible for developing and implementing policies and procedures, promote & advance Grove City Senior Living's Core Values & Philosophy of Care.

ESSENTIAL POSITION FUNCTIONS

· Facilitates an environment that promotes the optimal well-being and maintains the highest level of functioning for each resident.

· Collaborates with Wellness Director and Executive Director to ensure appropriate care is provided while maintaining daily routine and specialized program.

· Completes and implements individualized service plans. Maintains documentation and assessments to meet state regulations and resident needs.

· Provides orientation, training and education to new residents, families, and employees regarding Memory Care.

SPECIAL REQUIREMENTS

· Must be willing work flexible hours, including some evenings and weekends due to special events. Manager on Duty rotation is required.

Education and Qualifications

LPN license required

· Preferred experience working with residents with memory loss in a long-term care environment or assisted living community.

· Preferred experience of serving in a lead or supervisory position.

Experience:

• Memory Care: 1 year (Required)
• LPN license
